Holistic Assessment of the care recipient:
This specialised assessment utilises two criteria to assess the care recipient.

  • The de Morton Mobility Index (DEMMI) to quantify capacity under 5 domains: bed, chair, static balance, walking and dynamic balance. This provides a score that represents the accurate functional capacity of the participant.
  • The O’shea No-Lift System and criteria to determine the capacity of the participant, their ability to assist and ultimately the level of assistance required to assist them. The assessment considers all of the different transfers and movements that take place during a day, including transfers on the bed as well as off the bed, transfers that involve transferring from sitting-to-standing as well as ambulation.

Recommendations:

  • The outcomes of these assessments are recorded in a detailed report which can provide client specific recommendations in relation to the capacity and care requirements at that time.
  • This assessment is not a set & forget process, it is a living document and will require review if capacity changes with the participant.

Outcomes

  • Ensures Safety of both the Caregiver & the Participant:
    While the primary focus of caregiving is often on the care recipient, the well-being of the caregiver is equally crucial. Once completed, the Lift Less Functional Capacity Assessment dictates which transfers & procedures that will be safe & effective for the carer. This “empowers a safe care partnership” between the caregiver and the participant.
  • Tailored Care Plans:
    The insights gained from the assessment pave the way for the development of highly individualised care plans. Unlike generic plans, these are crafted to address the specific needs and limitations of the participant, promoting a targeted and effective approach to caregiving.
  • Promoting Long-term Well-being:
    This assessment not only addresses immediate needs but also looks toward the future. This proactive approach prevents injury, burnout and fatigue, enabling a sustainable caregiving dynamic.
  • Identifies opportunities for further Training & Education:
    The assessment identifies gaps in care and highlights opportunities where further one to one training can facilitate better outcomes for both the participant and the caregiver.
